SOC-445: The Case Management Process
Assignment Criteria
As a social worker, it is imperative to know and apply the case management process. The purpose of this course is to prepare students to achieve these objectives. For this assignment, use the Case Management Process table of criteria below to complete the Case Management Process template located in assignments folder in LoudCloud:
First, select one client/consumer from the course videos or case studies listed below. All materials are located in Topic 1 folder in MindTap:

Watch the corresponding video or read the selected case study.
For this assignment (Due in Topic 7) you will access and complete the Case Management Process template.
This assignment contains multiple parts that are due at different intervals in the course, with the final assignment due in Topic 7.
Next, begin to develop the initial plan by completing the following sections:
Section 1: Assess the Client/Consumer Needs (150-200 words) Due in Topic 2
Section 2: Assess the Client/Consumer (250-300 words) Due in Topic 3
Section 3: Planning and Linking (500-750 words) Due in Topic 5
Section 4: Intake and Monitoring (300-350 words) Due in Topic 6
Section 5: Terminating the Case (300-350 words) Due in Topic 7
Complete each section of the template (1,500-2,100 total words).
Cite three to five sources including the textbook.
Terminating Intake & Monitoring Planning& Linking Assessment | Section 1: Assess the Client/Consumer Needs (150-200 words) Due Topic 2
Review Chapters 1-2 in the textbook. Review Figure 1.1 in Chapter 1. For this section, explain the presenting problem and describe the identified problem. Turn in Section 1 at the end of Topic 2 for instructor feedback. Section 2: Assess the Client/Consumer (250-300 words) Due Topic 3 Review Chapters 3-4 in the textbook. For this section, explain the client’s micro, meso, and macro level systems, developmental transitions, and cultural impact, including culture and possible subculture. Turn in Section 2 at the end of Topic 3 for instructor feedback.
Section 3: Planning and Linking (500-750 words) Due Topic 5 Review Chapters 5-6 and 20 in the textbook. For this section, begin to develop the initial plan that includes the following: Consider possible interventions. Explain the systems affecting the client/consumer. Conduct the strengths perspective and explain how you would use this assessment to assist the client/consumer. Recall what you learned about linking the client/consumer to resources (Chapter 1 in the textbook) and recommended resources, while taking into consideration the stakeholders and interventions, and explain possible barriers to resources that may exist. Turn in Section 3 at the end of Topic 5 for instructor feedback. Section 4: Intake and Monitoring (300-350 words) Due Topic 6 Review Chapters 7-9 and 15 in the textbook. Keeping in mind empathy, boundaries, and effective communication skills, compose an introduction and 10-15 intake interview questions you would ask to assess your client. Read Chapters 24-25 in the textbook. Next, provide a possible monitoring strategy (make sure to refer to your state’s regulations) including measurable goals, objectives, and the client/consumer’s role. Be certain to explain the client autonomy and how you will respect that autonomy. Explain your plan to handle a possible reluctance to change or collaboration your client/consumer may have. Determine appropriate interventions that align with specific client goals. Turn in Section 4 at the end of Topic 6 for instructor feedback. Section 5: Terminating the Case (300-350 words) Due Topic 7 Using the feedback you received from the instructor for each section of this assignment, revise each section and, lastly, provide the following: Read Chapter 26 in the textbook. Review the NASW Code of Ethics. Describe a possible termination process for your client/consumer and recommend ways to maintain change including preparing for a possible relapse, while keeping in mind the autonomy of the client. Once you have made all the revisions, complete the assignment using quality academic writing. Cite all sources. |
